BSA Health Record

The Scouting adventure, camping trips, high-adventure excursions, and having fun are important to everyone in Scouting—and so is your safety and well-being. Completing the Annual Health and Medical Record is the first step in making sure you have a great Scouting experience.

All Scouts must have a complete BSA Annual Health and Medical Record (Parts A and B) on file with us current within the previous year.  Parts A and B can be completed without a visit to a health professional.

Anyone attending summer camp must have had a physical examination within the year prior to the last day of camp, conducted by a health professional, and had the results of the examination transcribed onto the latest BSA Annual Health and Medical Record (Parts A, B and C).
